Dr. Neel Bulchandani found his way to the answer the hard way, after his daughter was born with a tongue tie and none of the fascial, cranial, or visceral work he knew would move it. Over the years that followed he checked the mouths of every baby, child, and adult in his practice and kept finding the same 70 areas repeating. Then he distilled those 70 down to four. Understand what you're creating:

The cascade that frees tense areas, tones weak ones, and settles the nervous system, why it behaves like a modulator rather than a release, and how to recognize it in the body in front of you.

Establish a baseline you can trust:

Screen the palate, lips, cheeks, floor of the mouth, and tongue elevation without treating them, then check the head, throat, sides, and feet, plus the adductors and calves, so every change you see afterward is measurable.

Work the four regions:

Parietal, sphenoid, cerebellar and sub-occipital contacts; thyroid, hyoid and manubrium; the lateral chain under the last rib; and the foot through dorsiflexion, plantar flexion, inversion and eversion, with the five-second hold that does most of the work.

Fold it into what you already do:

Pre- and post-frenectomy care, retrognathic mandibles, diastasis recti, hypersensitive gag reflex, mandibular tori, palate expanders, scar tissue, and reflex integration, including when the Floor Four is enough and when it isn't.

Dr. Neel Bulchandani, DC, is the creator of FMDI, Functional Mandibular Diaphragmatic Integration, and the Floor Four framework. A Palmer Davenport graduate, he moved into airway and feeding work in 2016 after his own daughter was born with a tongue tie and the fascial, cranial, and visceral work he knew wouldn't release it. He trained in craniosacral fascial therapy with Barry Gillespie, collaborates with release providers and airway institutes across the country, hosts the Compromised to Alchemist podcast, and teaches the Floor Four internationally.
